A client intake form gathers important info when clients schedule, pay, or contact your business. Email and first name are required, while last name and phone number are recommended. You can also add other fields to collect everything you need.
Add a Custom Field
Add a custom field to your intake form to collect the specific information you need from clients.
To add a custom field:
- Open the platform.
- Click Settings.
- Click Client & Contact Info.
- Click Intake Forms.
- Click a form to customize.
- Click Add client field or Add contact field.
- Select a field from the drop-down menu or click Create new project field.
- Optional: Enter the details if necessary.
- Click Add.
Note: The client fields will show up in the same order as they appear in the Client Fields tab.
Remove a Field
To remove a field:
- Open the platform.
- Click Settings.
- Click Client & Contact Info.
- Click Intake Forms.
- Click a form to customize.
- Next to the field, click the more options icon

- Click Remove.
Click Ok.
Add a Business Policy
Add business policies to each intake form to keep clients informed. Policies will appear before the form is submitted.
To add a business policy:
- Open the platform.
- Click Settings.
- Click Client & Contact Info.
- Click Intake Forms.
- Click a form to customize.
- Under "Business Policy", click Business Policy, Booking Policy, or Payment Policy.
- Add a description to explain your policy.
- Click Save.
Add a Terms of Service
When enabled, clients must accept your Terms of Service before submitting the form. They'll see a link to view the full terms on your website. This works best if your terms are already posted online.
To add a terms of service:
- Open the platform.
- Click Settings.
- Click Booking Options.
- Click Booking policies.
- Under "Terms of Service," check the box Requests users to accept my Terms of Service.
- Enter a valid URL.
Preview Intake Form
Previewing an intake form lets you see how your fields will appear to clients, so you can make sure everything looks just right.
To preview intake form:
- Open the platform.
- Click Settings.
- Click Client & Contact Info.
- Click Intake Forms.
- Click Preview.
